About the NSFHT...

The North Simcoe Family Health Team is a not-for-profit primary health care organization located in the town of Midland, Ontario. We have been operational since April, 2007. We are a community of health professionals that values and respects the dignity of all people. We strive to serve in a caring and compassionate manner, promoting the wholeness of the individual, emotionally, physically, psychologically, socially and spiritually.

The issues and problems currently facing clients and patients in the health care system are often very complex. Consequently, the notion of improving an individual’s health has many different dimensions. It is extremely difficult for an individual health care professional from any one discipline to provide truly comprehensive, holistic care. An inter-professional approach to treatment enables individuals from various disciplines to share unique perspectives to achieve a common goal – the augmentation of the current health status of a patient or client.

Thus, in order to address all aspects of a patient’s well being, health care workers must integrate information and work as a team. One solution to improving this much needed communication and collaboration between health care professionals is the model of health care delivery adopted by the North Simcoe Family Health Team.

Our Primary Health Care Team currently consists of:

• 14 Family Physicians
• 3 Nurse Practitioners

Patients registered to one of these physicians/NPs have access to the programs and services delivered by our four Nurse Educators, three Social Workers, Dietitian, Pharmacist and four Administrative Staff. Our Team offers mental health counseling, chronic disease management and health promotion programs. The physicians of the NSFHT also offer an after-hours episodic clinic.
