What is a Passport Clinic?

Since it is not always possible for people to come into one of my offices to get assistance with their passport applications, I’m bringing my office to you. ?At the clinic, you can receive assistance filling out the application, have your pictures taken, and have applications reviewed to ensure all materials and documentation are included and are ready to take to your guarantor for signing.

What Should You Bring to the Clinic?

¨ Proof of Citizenship - Canadian Birth Certificate, or Citizenship Card/Certificate (Original must be sent with application when applying for a child’s passport and for an adult general passport)

- A long-form birth certificate (Certified Copy of Birth Registration) is required for most Child applications

Birth certificates for children under 16 years of age - In accordance with new proof of parentage requirements, parents must provide with the child's application either a detailed (long-form) birth certificate (from Canada or a foreign country where the child was born) that indicates the names of the parent(s) or an order of adoption indicating the name of the adoptive parent(s). To obtain a detailed Canadian birth certificate, contact the vital statistics office of the province or territory where the child was born.

Proof of parentage

Effective October 1, 2012, a passport application for a child under 16 years of age must include one (1) of the following proof of parentage documents that demonstrates the child-parent relationship:

  • A detailed (long-form) birth certificate issued by a Canadian provincial or territorial vital statistics office that indicates the name of the parent(s) This document is also accepted as proof of Canadian Citizenship;
    • If born in Ontario, a certified copy of birth registration is acceptable.
    • If born in Quebec, a copy of an act of birth is acceptable.
  • A court-issued adoption order indicating the name of the adoptive parent(s); or
  • A foreign birth certificate that indicates the name of the parent(s)

    Note: documents in a language other than English or French must be translated into English or French by a certified translator.

Exception: A proof of parentage document is not required with an application only if a Canadian passport or travel document has previously been issued to the child and it is the same parent(s)/legal guardian(s) who applied for the previous passport or travel document participating in the new application process. All other documentation is still required, including proof of citizenship (however, in this case a short-form or wallet-sized birth certificate that doesn't indicate the name(s) of the parent(s) is acceptable).This new requirement enhances the security of a child’s passport.

¨ Additional Document to Support Identity - Valid Government-issued ID with a photo and a signature (e.g. Driver’s Licence, adult Health card, Old Age Security Card, S.I.N. card with a signature on it) Only a photocopy signed by your guarantor, of both sides of this secondary document is required. We will have a photocopier at the clinic for this purpose.

¨ Contact information for two references (non-relatives who have known you for at least 2 years) and One Emergency Contact. References do not need to sign your application.

¨ Current Passport (if you have one)

If you are getting your first passport, or renewing a passport that is more than one year past the expiry date, you will need a Guarantor - a person (can be a family member), who has known you for at least two years and who holds a valid adult Canadian Passport). Your guarantor will need to be present at the passport clinic with their valid passport in order to complete all the necessary steps.

The Guarantor is required to:

Sign the back of the stamped passport photo

Complete Section 2 of your Application

Sign any photocopies of the documents in support of your identity

If your Guarantor can attend the clinic with you, you will be able to leave your completed application at the clinic. If not, you may complete and have checked, all remaining requirements (other than the Guarantor) of the passport application, gather your Guarantor signatures afterward, and return the application to our office for shipping to Ottawa.

A photographer will be present to take pictures for $22 (Cash and Credit Card only)

 

Fee’s for Passports effective July 1, 2013

ADULTS $120 - 5 year, $160 - 10 year, CHILDREN $57 - 5 year only.

Payment must accompany the application either by credit card, certified cheque or money order.

Certified cheques or money orders must be made out to the Receiver General for Canada
