Community Living Huronia Programs

The mandate of the Foundation is to raise funds, invest these funds and expend funds solely to the Association of Community Living Huronia. Community Living Huronia exists to support the needs of the people with exceptionalities in the community. The services that are offered by Community Living Huronia which CLH Foundation assists to fund include:

 

Preschool Resource Services - Community Living Huronia Preschool Resource Services works with families who have children aged two to school entry age when there is a concern about their development.  Preschool Resource Services are provided free of charge and is funded through the County of Simcoe.
Resource Consultants help children whose parents are concerned about their child’s physical, social, emotional, or communication development.  Staff are specialists in early learning and child development.  Their goal is to help children meet developmental milestones and build a solid foundation for success.
Supports are family centered and based on the needs of the child and their family.  Supports respect the values and choices of the family, build on the strengths of the child, and work in partnership with the family.  We strongly encourage and promote inclusion within the community.



 
 
Community Activity Program Support (CAPS) – CAPS offers a wide range of opportunities for adults based on their interests and needs.  Trained, experienced staff support the individual to explore as many community based activities and experiences as possible.  The support staff work closely with Community Partners to provide quality services designed to meet the needs of the individual.  CAPS believes that all individuals should have the opportunity for life-long learning, whether through experiential or formal education and learning opportunities.  The program also offers Recreation Services that provide individuals with a broad range of social and recreational opportunities in and around the community.  The program endeavours to plan and execute activities, while bringing like minded people together.
 
Supported Employment - Assists individuals 18 years of age and over explore/obtain and/or maintain employment opportunities. Our Support Staff work directly with the individual and the potential employer to ensure mutual satisfaction. We connect potential employers with suitable employees.  We also offer an 8 week program designed to help job seekers prepare for competitive employment.  We provide in class modules on resume writing, job searching, safety, and more.  Included in the 8 week program is on the job training placements, and tours of local industries, restaurants and facilities to educate the job seeker in the different types of employment that could be available to them.
Residential Services - Community Living Huronia supports 84 people in 20 residential settings throughout North Simcoe, including Penetanguishene, Midland, Port McNicoll, Waubaushene, and Victoria Harbour.  In addition, four individuals are supported in the Family Home Program, and 34 people in the Supported Independent Living Program (SIL).  Four of these individuals receive specialized supports through dual-diagnosis SIL services.  The type of support an individual requires depends on their specific needs.  Some people live on their own with minimal support from staff, others live with families, such as in the Family Home Program, while still others reside in group homes that are staffed on a 24 hour basis.  Community Living Huronia’s emphasis is focused on the independence of individuals with exceptionalities, and supplying the supports needed to maintain or enhance each individual’s independence.  
Future Transitions Program - The Future Transition Program operated by Community Living Huronia provides support to young people with special needs between the ages of 16 to 21 in their school to adult life transition.  The Future Transition Program Coordinator acts as a support and resource to the teen, their family, school and community agencies.  The interests of the young adult are explored, sills and abilities are assessed, and plans are made through the Person Directed Planning model to assist in meeting goals for the future.  Supports and resources are put into place based on individual preferences and interests.  
Teen Camp is a program solely funded by donations that assists youth with skill building, opportunities to build confidence and friendships, and explore interests.  This program works in partnership with the Future Transition Program.
